Henry Community Health boosts annual wellness visits with Oracle Health

Henry Community Health, an integrated health system serving east central Indiana, needed better data insights to identify patients eligible and due for Medicare Annual Wellness Visits (MAWV). To address this challenge, the organization turned to Oracle Health Data Intelligence and Oracle Health Enterprise Data Warehouse, along with Oracle Health EHR and Oracle Health CRM Connector, to improve patient identification and outreach.

Oracle implemented a custom MAWV Discovery Dashboard that helped Henry Community Health track appointments and reach the right patients at the right time. As a result, MAWV completions rose from 66% of eligible attributed patients in 2022 to 76% in 2023, moving the organization closer to the 80% target for 2024.
