UnitedHealth Group faced the challenge of coordinating care for older adults who often have multiple chronic conditions, social and emotional needs, and environmental risks that threaten their ability to stay healthy at home. Optum360, working with AARP, launched pilot Case Management programs offering integrated clinical and behavioral services, telephone care managers, social workers, and face‑to‑face visits to improve care coordination and quality for this population.

Optum360’s solution combined targeted outreach, evidence‑based metrics, and in‑person assessments to manage chronic disease and address fall risk and other quality‑of‑life issues. By year two the program showed increased quality of care: engaged members were more likely to maintain or increase medication compliance and diabetics were significantly more likely to have an annual office visit; a fall‑risk screening of 1,645 AARP members led to 75% having no further falls that year.
