Pioneer ACO Brown & Toland Advances Patient-Centered Accountable Care Supported by Population Analytics from Optum One

Brown & Toland, a 1,500-physician independent practice association and Pioneer ACO serving over 335,000 patients in the San Francisco Bay Area, sought to identify high‑risk patients and continuously improve care coordination as it built My Health Medical Group (MHMG), a patient‑centered medical home. To achieve that, Brown & Toland partnered with Optum360 and deployed Optum One Population Analytics to turn EHR and billing data into actionable population insights.

Optum360 implemented Optum One Population Analytics for risk stratification, predictive analytics, working registries and performance dashboards, enabling care teams to target outreach, redesign workflows (same‑day/Quick Sick visits and after‑hours scheduling) and track team performance. The results included dramatic access improvements—average appointment wait time fell from 13.5 to 4.3 days—performance at or above the 75th percentile across measures, stronger follow‑up for high‑risk patients, and support for MHMG’s NCQA Level 3 recognition.
