National U.S. Employer, a company with over 40,000 employees that publicly prioritizes employee health, launched a voluntary incentive program offering up to a 10% premium discount tied to four biometric targets (tobacco-free status, BMI <24, total cholesterol <150 mg/dL, and blood pressure 110/70). To operationalize the program and meet an aggressive three-month rollout across more than 300 worksites, the employer contracted Optum360 to administer comprehensive biometric screenings and manage incentive qualification.

Optum360 delivered on-site venous draws and testing (full lipid panel, fasting glucose, cotinine, BMI/WHR and blood pressure), completed initial screenings within the three-month window, and reported results to participants. Of ~40,000 eligible employees, 13,025 were screened in 2011 and 65% (8,414) earned an incentive; population-level improvements included total cholesterol down 5%, TC/HDL ratio down 16%, glucose down 9%, BMI down 7% and triglycerides down 30%. Optum360’s program also drove higher wellness coaching engagement (touched: 3,978→4,748; actively engaged: 845→1,658), demonstrating measurable risk reduction and increased program participation.
