Mid Hudson Medical Group, a 125‑physician practice across 16 Hudson River Valley locations, sought better clinical intelligence to advance its patient‑centered medical home (PCMH) model. Facing gaps in longitudinal patient data and the ability to track population and individual health measures, Mid Hudson turned to Optum360 and its Optum Population Analytics (formerly Humedica MinedShare) to consolidate EHR and other system data for population health management.

Optum360 implemented Optum Population Analytics to deliver continually refreshed, longitudinal clinical data, clinic dashboards and risk‑stratification tools that enabled targeted outreach and benchmarking. As a result, about one‑third of identified diabetic patients were seen within eight months, one‑third of that diabetic cohort achieved an HgA1c <8%, and 60% of those with HgA1c >9% are now under intensive management — metrics that have become integral to Mid Hudson’s PCMH care coordination and quality improvement.
