HealthEast’s Twist on Medical Homes Demonstrates Broad Range of Successes

HealthEast Care System, the largest provider in Minnesota’s East Metro, launched a Health Care Homes (medical home) initiative to better engage patients, coordinate care and measure outcomes across disparate EMR and claims systems. To identify high-risk patients and track clinical and utilization trends, HealthEast partnered with Optum360, using Optum One Population Analytics and Predictive Risk Modeling to aggregate and normalize data for analysis and outreach.

Optum360’s analytics platform enabled HealthEast to target cohorts for intensive care coordination delivered by certified Care Guides, and to create a single “source of truth” for outcomes tracking. As a result, HealthEast saw an average 30% reduction in emergency room visits across cohorts, a 39% reduction in hospital admissions, a 4% average drop in mean A1C and up to 2.5x increase in diabetes education—measurable impacts driven by Optum360’s data aggregation, predictive modeling and reporting.
