Colorado Springs Health Partners Drives Patient-Centered Medical Home with Analytics from Optum One

Colorado Springs Health Partners (CSHP), a physician‑owned multispecialty practice serving the Pikes Peak region, needed trusted clinical and operational data to advance its Patient‑Centered Medical Home (Patient Care Compass), engage clinicians, and manage population health for chronic conditions like hypertension and diabetes. To build an analytics capability that supports PCMH goals, meaningful use and CMS initiatives, CSHP turned to Optum360 and its Optum One Population Analytics solution.

Optum360 implemented Optum One Population Analytics to aggregate and normalize EMR, practice‑management and claims data into longitudinal patient and population views. Using those insights and clinician engagement led by CSHP’s CMO, the organization improved care coordination after hospital and ED visits, achieved an interim hypertension control rate of 70% within a year (moving toward an 80% target), and recorded clinic‑level hypertension control improvements ranging from 3.5% to 19.8%, while enabling benchmarking and reporting to CMS and other payers.
