Excel Medical Center, a Philadelphia network of 14 primary care offices with about 25 physicians and nurse practitioners serving roughly 20,000 members (65% dual Medicare/Medicaid), struggled with interoperability failures, outdated workflows, burdensome form management and limited staff capacity across multiple payer contracts. To address these issues, Excel enrolled in the Optum In-Office Assessment Program and partnered with Optum to integrate the program’s capabilities into their EHR.

Optum integrated its In-Office Assessment Program with Excel’s EHR and assigned a field team/health care advocate to customize workflows, provide monthly support, and enable actionable analytics, point-of-care alerts, automated submissions and chart extraction. The solution streamlined workflows, saved staff time, supported planned expansion and improved clinical outcomes—illustrated by an early detection of worsening peripheral vascular disease that avoided emergency care—and leverages Optum’s suspecting logic, reported at 98% accuracy in flagging conditions.
