South Karelia Social and Healthcare District (Eksote) is a regional healthcare and social services organization formed in 2010 that serves nine municipalities and about 132,000 residents. Eksote faced limited data visibility beyond central patient records and complex, manual coordination among many care stakeholders, which created bottlenecks—most notably long hospital discharge times—hindering timely, joined-up care across units.
Chainalytics implemented a user-led, agile pilot using Micro Focus Solutions Business Manager (SBM), going from prototype to full rollout in two months and integrating processes with Eksote’s electronic medical records. Automated workflows connected discharge queues to health centers and coordinated multi-stakeholder care, clearing backlogs and improving transparency. Today some 3,500 users benefit from streamlined processes, better collaboration, longer home care for elderly patients, and a recognized best-practice model in Finland.
