Carver County HHS was struggling with manual, disjointed paperwork processes that slowed releases of information, assessments, and support plans from weeks or months to a crawl. The delays disrupted services for residents, created stress for workers, and made it difficult to manage forms securely in a hybrid work environment.
Northwoods implemented Traverse and its client portal, Traverse Connect, to let staff share forms, collect signatures, and automatically route completed documents into electronic case files from anywhere. The result was faster collaboration and better continuity of care, with some tasks dropping from two weeks to 30 minutes and others from two months to two days, while also reducing stress and keeping forms secure and up to date.
