Bellin Health, a next generation accountable care organization (ACO) with hospitals and primary care clinics in Green Bay, WI, wanted to launch a chronic care management (CCM) program to improve care coordination, patient outcomes, and patient targeting while working within its existing care team structure. Nordic partnered with Bellin to support the project using Epic’s Healthy Planet Foundation System build as the starting point.

Nordic designed and implemented compliant CCM workflows to identify eligible patients, streamline enrollment, monitor care plans, and automate charging. The results were strong: the project went live on time in June 2012, was significantly under budget, and Bellin’s Epic application teams were self-sufficient within three months; in the CCM program, Nordic helped Bellin produce results within weeks of kickoff and improve tracking and operations through better workflow design.
