Terros Health Inspires Change for Life with Integrated Care

Terros Health is a nonprofit community health organization founded in 1969 with 15 locations, 900 employees, and a mission to deliver whole‑person care to more than 50,000 patients annually. As Terros moved from behavioral‑health–only services toward integrated primary and behavioral healthcare, leaders found their existing behavioral EHR did not support primary care workflows and contributed to fragmented records and an unwieldy set of forms.

Terros implemented an integrated EHR and care model using NextGen solutions, colocating primary and mental‑health providers, streamlining single check‑ins, and adding services such as a minute clinic in a probation office. The change reduced behavioral health forms from about 700 to 200, created unified patient records, supported eight integrated care centers (four FQHCs), and drove improved outcomes for asthma, hypertension, depression, and diabetes while positioning the organization to grow primary care and specialty services.
