Neighborhood Health Leverages Mobile to Reinvent Street Medicine

Neighborhood Health is a nonprofit network of 11 community health centers in Nashville serving more than 31,000 patients and managing over 90,000 visits a year. To reach an estimated 2,000 people experiencing homelessness, the organization formed four multidisciplinary street medicine teams to deliver on-site primary, behavioral, prenatal and urgent care—conducting roughly 700 unique street encounters in a year—and needed a way to provide that care while preserving clinical efficiency, documentation, billing, and care continuity.

Neighborhood Health equipped teams with NextGen solutions (Enterprise EHR/PM, NextGen Mobile Plus, Remote Scribe Services, and Patient Self‑Scheduling) plus mobile diagnostics (Bluetooth stethoscope, handheld ultrasound, mobile ECG) to document visits in real time, dispense meds, schedule follow‑ups, and integrate charges with HMIS and grant billing. The approach preserved workflow and billing efficiency, saved clinician time through remote scribing and mobile documentation, increased access to care, built patient trust that led to clinic follow-ups, and enabled reliable tracking and reporting of outreach services.
