First Care Clinic is a rural Federally Qualified Health Center and Level 3 Patient-Centered Medical Home in Hays and Victoria, Kansas, offering medical, dental, and mental health services under one roof. Facing the typical FQHC challenge of doing more with less—serving patients who often travel long distances and frequently need multiple services in one visit—the clinic needed a way to proactively identify clinical and financial needs before appointments to reduce delays, errors, and missed care opportunities.

Partnering with NextGen Healthcare, First Care implemented an integrated stack (Ambulatory EHR, Practice Management, Patient Portal, Electronic Dental Record, Revenue Cycle Management, and NextGen Share) and a data-driven workflow that begins with daily multidisciplinary huddles and pre-visit EHR reviews. The integrated system centralizes records, streamlines dual scheduling and accurate payment posting, shares information in real time across departments, and resulted in more efficient, coordinated visits, improved clinical and financial outcomes, and higher patient satisfaction.
