Integrated solutions bring care to underserved population

Grace Community Health Center (GCHC) is a federally qualified health center serving medically underserved, rural communities across six locations in southeastern Kentucky. Dependent on HRSA grants and subject to strict Uniform Data System (UDS) reporting, GCHC faced challenges delivering continuous, whole-person care—primary, behavioral and dental—to a dispersed population that otherwise would rely on distant, fragmented and costly ER care.

By implementing NextGen’s integrated suite (Ambulatory EHR, Practice Management, Health Quality Measures, Care, Patient Portal, Mobile and QSIDental), GCHC unified patient records, streamlined UDS reporting and coordinated care across clinics and school programs. The integrated solution helped GCHC triple in size over 18 months, stay current with reporting requirements, provide more proactive care—including life‑saving school-based interventions—and strengthen its ability to secure federal funding.
