A NextGen Healthcare Case Study
Partners in Recovery (PIR) is a Phoenix-based integrated medical and behavioral health network founded in 2009 that serves adults with severe mental illness (SMI). Faced with the shift to value-based, whole-person care, PIR needed to reduce high ER utilization and address the clinical and social drivers of poor outcomes—smoking, substance use, medication nonadherence, comorbid chronic conditions, low health literacy, homelessness and fragmented care—especially among its highest-cost/high-need patients.
PIR implemented a multidisciplinary, clinic-based model that co-locates primary care, psychiatry and wellness services, uses a High-Risk “Familiar Faces” registry and functional risk assessments, and leverages the NextGen Behavioral Health Suite to aggregate EHR, claims and HIE data for targeted interventions. The approach improved access, coordination and efficiency, expanded peer and family supports, and more than doubled PIR’s revenue and patient population (from 3,800 to about 8,000 across seven clinics) while sustaining 8–10% annual growth.
