A NextGen Healthcare Case Study
Blackstone Valley Community Health Care is a community health center serving densely populated, predominantly Latino Central Falls, Rhode Island, where poverty and limited access to care left residents underserved and the clinic constrained by a small facility and the recent loss of the local hospital. Leadership sought to expand capacity and develop a data-driven way to reach and manage care for nearly the entire community.
In 2019 Blackstone Valley opened a 47,000 sq ft neighborhood health station that co-locates primary care, dental, behavioral health, radiology, rehab and pharmacy services and uses NextGen Population Health analytics to combine claims and clinical data for pre-visit planning and risk stratification. The result: a single-practice accountable entity serving about 17,000 residents with ~100 staff and 80–90k visits/year, improved care coordination and targeted outreach (e.g., smoking cessation), and a scalable model for population-level health improvement.
