Southern California's Medical Group faced a fragmented data environment with a data warehouse of over 2 million patient files that lacked unique patient identifiers and was about to double in size due to a large acquisition. They also needed a centralized, consistent repository of provider and organization information to simplify referrals and integrate physicians into the admission process. The group engaged NextGate and its EMPI patient matching along with Provider & Organization Registry solutions to address these challenges.

NextGate implemented its Enterprise Master Patient Index (EMPI) to create unique patient identities and deployed the Provider & Organization Registry to assign unique provider IDs and consolidate provider data. As a result, NextGate enabled a consolidated data warehouse of uniquely identified patients, patient health plan ID tracking across systems, accelerated physician involvement at admission, expedited referrals, and improved relationships with participating organizations.
