The Fortune Society, a nonprofit that helps formerly incarcerated people re-enter their communities, serves more than 6,000 clients with a $28M budget and offers integrated services across housing, health, employment and education. It needed to consolidate data from many internal and external systems (including EHRs and state exchanges) to meet reporting and reimbursement requirements, streamline grant funding, and gain a holistic, timely view of client needs and outcomes.
