Advocate Aurora Health, a large U.S. healthcare network serving nearly 3 million patients across 500 sites, needed a secure, centralized way for interdisciplinary teams to coordinate care, reduce readmissions, and lower administrative burden. To solve this, Advocate Aurora Health partnered with Microsoft to pilot a care coordination solution built on Microsoft Teams, integrated with Office 365 apps like SharePoint Online, Exchange Online, and Yammer.
Using Microsoft Teams as the collaboration hub (with adoption support from Slalom Consulting), Advocate Aurora Health gave IDTs persistent chat, meetings, shared notes, and secure access to patient information while maintaining HIPAA compliance through Office 365 security tools. The pilot improved real-time communication and visibility, reduced time spent searching emails, helped avoid unnecessary higher-level care, and expanded from Advocate Sherman Hospital to another hospital with plans to add two more by the end of 2018—demonstrating strong adoption, lower TCO, and measurable operational impact.
