New York’s largest healthcare provider streamlines patient care processes with Microsoft business applications

Northwell Health, New York State’s largest healthcare provider, faced the challenge of shifting to value‑based care and ensuring timely, coordinated discharges while giving clinicians a 360° view of patients. To replace paper‑based workflows and improve information agility, Northwell adopted Microsoft Power Apps and Microsoft Dynamics 365 (including Dynamics 365 Customer Service, Common Data Service, and the Healthcare Accelerator), alongside Microsoft Teams and Power Automate.

Using Microsoft Power Apps, Northwell rapidly developed a daily rounding app (built in weeks by clinicians) that consolidates patient data, tracks discharge tasks, and pushes proactive readmission alerts via Power Automate and Teams; the app now integrates AI with the Microsoft Bot Framework. The Microsoft Power Apps‑based solution improved workflow visibility, streamlined discharge coordination, reduced developer time spent on HIPAA/compliance, and set a scalable foundation for more digital care and patient‑engagement apps during initial deployments at select hospitals.
