VITAS Healthcare, a leading US hospice care provider serving patients across 14 states and the District of Columbia, faced time‑critical, paper‑based patient charting and manual clinician scheduling that demanded hours of coordination for weekly interdisciplinary group (IDG) meetings. To centralize care coordination and reduce delays in delivering end‑of‑life care, VITAS partnered with Microsoft Power Apps and the broader Power Platform, integrating solutions into Microsoft Teams.

Using Microsoft Power Apps to build "Emmy" — a suite of Power Apps canvas apps nested in Teams with Power Automate flows, Power BI reporting, and Graph API integrations — VITAS eliminated paper workflows and enabled real‑time scheduling and triage. The Microsoft Power Apps solution cut manager preparation from hours to minutes, captures 92% of patient visits, supports about 60,000 visits per week, has hosted 300 IDG meetings weekly, and processed more than 2 million visits since June 2021, improving responsiveness and clinician efficiency.
