chemo@home streamlines mobile medical staff coordination for at-home treatments with AvantCare, based on Microsoft Dynamics 365

chemo@home, an Australian at‑home cancer‑treatment provider serving more than 7,400 patients, faced coordination and connectivity challenges across a widely distributed mobile nursing workforce, inconsistent devices, and rising support costs. To modernize operations they engaged Illuminance Solutions to deploy AvantCare — a Microsoft Dynamics 365–based platform augmented by Azure, Teams, Power BI and Microsoft Power Apps — creating a single, mobile‑accessible system to match staff qualifications to patient needs and ensure equipment availability.

The Microsoft Power Apps‑enabled AvantCare solution gave field teams real‑time access to patient records, scheduling, inventory and secure device management, while Dynamics 365 Field Service and Customer Service matched work orders to appropriately qualified staff. As a result, all chemo@home team members can connect on any device, critical patient information and reporting have significantly improved, administrative time has been redirected to patient care, and the service scaled rapidly to meet COVID‑19 demand — supporting continuity of care for thousands of home‑based patients.
