Outcome-based healthcare provider uses cloud-based population health management to expand new care model

AiR Healthcare Solutions aimed to shift behavioral-health treatment from episodic crisis care to a value-based, population-health model by launching its AiRCare disease-management service. To scale personalized, secure care coordination and analytics it deployed Microsoft technologies—Dynamics 365, Azure, Azure Active Directory—and used Microsoft Power BI for reporting and visualization.

Using Tribridge’s Health360 Care Coordination built on Dynamics 365 and Azure with Microsoft Power BI dashboards, AiR Healthcare Solutions coordinated outreach, personalized care plans, and real-time clinical insights. Results included 62 of 124 high-risk patients moving to moderate risk and 24 to low risk within six months (an estimated ~$2M annual savings for one population), a 72% continuous-abstinence rate versus a 35% national average, reduced readmissions, and plans to scale AiRCare to serve 250,000 people—all enabled by Microsoft Power BI–driven analytics.
