Solera Health is a Phoenix‑based healthcare technology innovator that connects health plans, community diabetes prevention programs (DPPs), and patients to prevent type 2 diabetes. Faced with a fragmented network of roughly 1,700 DPPs and the need to securely aggregate and analyze sensitive member data at scale, Solera adopted a cloud-first approach using Microsoft Azure and Office 365 and analytics driven by Microsoft Power BI to identify people at risk and coordinate best‑fit prevention services.

Using Microsoft Power BI (with ArcGIS Maps for Power BI) on a secure Azure backbone, Solera built dashboards and heat maps to pinpoint high‑risk populations, match members to nearby DPPs, and monitor system health and program outcomes. The cloud solution helped Solera achieve HITRUST certification faster, increased insurer trust, and delivered measurable health impact—particularly among seniors—with participants reporting 15–20 pound weight loss in many cases and demonstrable reductions in service costs for health plans.
