Advancing Connecticut’s Data-to-Care program to stop the spread of HIV

The Connecticut Department of Public Health faced a critical challenge running its Data‑to‑Care HIV prevention program: antiquated lab reporting and siloed datasets made it difficult and slow to surface out‑of‑care cases from more than 70,000 lab reports a year. To modernize workflows and reduce manual matching and custom code, the state worked with partner Tallan and technologies including Microsoft Power BI to consolidate data and speed identification of people who might benefit from intervention.

Tallan developed the HANK system using Microsoft SQL Server, .NET Core/MVC and Microsoft Power BI, adding a Lab Staging module and a comprehensive HANK‑Person View to automate matching and reporting. The solution (HANK 1.2) integrated three systems and delivered measurable gains: 50% fewer staff hours for record searches, 50% less duplicate data entry, 75% fewer data‑entry errors, a 25% improvement in acute case detection, daily—rather than quarterly—Data‑to‑Care event identification, and reduction of out‑of‑care list generation from two months to zero days.
