Telecare support for Polish elderly and disabled enhances quality of life

The Polish Medical Association launched a pilot telecare program in the Greater Poland region to help elderly, disabled and vulnerable citizens live more independently while better connecting patients, carers, clinicians and social services. Facing the challenge of real‑time monitoring and coordinated response across many stakeholders, the Association needed a platform that could manage patient data, alerts and resource allocation at scale for its 550‑person trial.

The solution uses tightly integrated wearables, phones and a Microsoft Dynamics CRM‑based back end to capture 24/7 monitoring data, flash urgent or non‑urgent alerts to contact center staff, and route emergency responses or follow‑up support. Early results show improved response times, more flexible resource allocation through detailed reporting and analytics, and a measurable boost in quality of care and independence for participants.
