VitruCare, developed by Dynamic Health Systems for a group of General Practitioners in Bradford, uses Microsoft Azure and Dynamics CRM to help patients self-manage long‑term conditions. Facing the twin pressures of improving care and cutting costs under constrained NHS budgets, the practice needed a way to reduce unnecessary visits while keeping clinicians informed and patients engaged.

The VitruCare app lets patients track symptoms, log vitals (blood pressure, weight), set personal goals, keep a health diary and securely message their care team, with granular control over who sees their data. Adopted across the Bradford practice and in hospice settings, the solution delivered overwhelmingly positive user feedback, a 40% reduction in patient visits and more than £1 million in annual savings, with wider uptake by other providers underway.
