Outcome-based healthcare provider uses cloud-based population health management to expand new care model

AiR Healthcare Solutions, a Minnesota–based behavioral health provider, set out to move treatment from episodic crisis care to a value-based, population-health model that promotes sustained patient engagement and accountability. The organization aimed to address the high readmission rates, poor long-term outcomes, and mounting costs associated with treating chronic behavioral disorders as isolated crises.

AiR launched AiRCare, a cloud-based disease-management service built with Tribridge Health360, Microsoft Dynamics 365 and Azure (including Azure AD and Power BI), to coordinate personalized long-term care, secure data, and deliver actionable analytics. The platform enabled scalable outreach, risk stratification, and reporting, producing strong results: within six months 62 of 124 high‑risk patients moved to moderate risk and 24 to low risk (saving an estimated $2M annually for one population), continuous abstinence rose to 72% versus a 35% national average, and the service was projected to serve 250,000 people.
