Mount Sinai Health System, a leading healthcare provider, faced the challenge of improving care coordination for low-income populations in New York as part of a state Medicaid initiative (DSRIP). To participate, they needed to create a massive network of over 200 diverse provider organizations and a secure, flexible shared technology solution that could meet stringent regulatory requirements. The solution implemented by Microsoft Azure was a massive, secure hybrid cloud, one of the largest in healthcare. It provided a central portal and applications for over 10,000 providers, enabling improved care coordination, chronic-disease management, and real-time performance analytics. As a result of this IT transformation with Microsoft, Mount Sinai expects to improve population health and reduce avoidable hospital use by 25 percent over five years.
