Patients take control of their own care with VitruCare

VitruCare is a patient-centred care app developed for a group of general practitioners in Bradford who faced the twin pressures of improving outcomes and cutting costs under shrinking NHS budgets. The GPs asked Dynamic Health Systems to build a solution that would enable patients to monitor symptoms, set personal health goals and share data with clinicians to reduce unnecessary practice visits.

Built on Microsoft Dynamics CRM and Azure, the VitruCare app lets patients log symptoms and vitals (blood pressure, weight), track goals and diary entries, and securely message clinicians while controlling who sees their data. The result: overwhelmingly positive user feedback, a 40% reduction in patient visits at the Bradford practice, and more than £1m in annual savings — outcomes that are driving wider adoption across other healthcare providers.
