Outcome-based healthcare: provider uses cloud-based population health management to expand new care model

AiRCare, the cloud-based disease management service from AiR Healthcare, was created to shift behavioral health treatment from acute crisis care to a value-based model focused on long-term accountability and sustained patient-provider communication. To support this goal, AiR Healthcare needed a scalable way to manage population health, personalize care, and handle complex clinical data while meeting security and compliance requirements.

Microsoft Azure, together with Dynamics 365, Azure Active Directory, and Power BI, was implemented through Health360 Care Coordination to coordinate care, generate insights, and streamline reporting. The solution helped AiR Healthcare scale to an expected 250,000 people, move 62 of 124 high-risk patients to moderate risk and 24 to low risk within six months, and raise 12-month continuous abstinence to 72% versus a 35% national average, while reducing healthcare costs by nearly $2 million a year for one population.
