AaNeel, a U.S. healthcare technology company, faced the challenge of fragmented records and slow communication across managed care networks—issues that lead to delayed notifications (PCPs learn of ER admissions on average 2.68 days later), medication errors, and gaps in care. Large networks such as IntegraNet Health, with 1,250 independent physicians and more than 25 EMR systems, needed a way to close those gaps and enable a true “Medical Home” model.

AaNeel built AaNeelCare plus a Patient Portal, AaNeel Connect (medication management), and an NFC Health Access Card on Microsoft Azure to provide real‑time, cross‑platform access, alerts, and HIPAA‑compliant hosting. The solution lets providers and patients share records instantly, notifies PCPs of ER visits, supports care coordination and follow‑ups, and has enabled customers like IntegraNet to improve care coordination, reduce errors, and close communication gaps.
