Waypoint Centre For Mental Health Care, a comprehensive tertiary mental health care teaching and academic hospital, needed a better way to manage care transitions. Paper Kardex hand-offs were taking 40+ minutes, creating frustration, duplication, and communication risks across inpatient units and support services. MEDITECH’s electronic health record and SBAR communication tool offered a way to standardize transfer-of-care processes and improve patient safety.

MEDITECH implemented an electronic SBAR workflow across 14 inpatient units, replacing paper during shift changes and pulling real-time risk and patient information into a concise, standardized format. The results included a drop in hand-off time to 10–15 minutes, immediate staff adoption, improved clinician and patient satisfaction, and no adverse events related to poor transfer of care since using SBAR.
