Kalispell Regional Healthcare, a 288-bed acute care hospital in northwestern Montana, was looking to shift from sick care to wellness care as it moved into CMS’s CPC+ value-based payment model. The organization struggled with patient attribution, follow-up, and gaps in preventive care, including overdue wellness visits, diabetic eye exams, and cancer screenings. To address these issues, Kalispell turned to MEDITECH’s Patient Registries.

With MEDITECH’s Patient Registries, Kalispell created actionable wellness, diabetes, and ED follow-up registries that helped staff identify at-risk patients, manage care gaps, and proactively reach out for needed services. The results included a 22% increase in colonoscopies, improved compliance with diabetic eye exams and wellness visits, and a rise in colonoscopy compliance from 52% to 74%. The registries also improved workflow efficiency, supported better follow-up after ED visits, and helped clinicians track key diabetes measures such as A1C, blood pressure, and foot exams.
