In Rural Texas, ‘Bridge to Health’ Mobile Clinic Uses MEDITECH’s EHR to Deliver Better Care

Palo Pinto General Hospital (PPGH), a rural Texas healthcare provider serving Mineral Wells and surrounding communities, faced the challenge of delivering care to patients who often lacked reliable transportation to follow-up appointments. To better reach this underserved population, PPGH needed a way to extend care beyond its central location while keeping records accessible across its outpatient network.

MEDITECH implemented Expanse Ambulatory for PPGH’s six clinics, including the Bridge to Health mobile clinic, creating a single, integrated patient record across the organization. With MEDITECH’s EHR, staff can access the same records at the urgent care center, schools, and mobile sites, improving communication, continuity of care, and operational cohesion as the clinic reaches patients in neighborhoods, schools, and community events.
