Making the Transition to Managed Care Easy for Individuals and their Families

Partners Health Plan (PHP), a not-for-profit managed care organization serving people with intellectual and developmental disabilities, needed a Medicaid managed care solution that supported day‑to‑day, person‑centered services rather than just medical problem‑based tools. After finding mainstream vendors inadequate, PHP engaged MediSked to provide a software solution that would connect individuals to needed services and supports, enable collaboration across interdisciplinary teams, and meet new government managed care regulations.

MediSked implemented its Care Coordination Suite — including MediSked Coordinate, Connect Exchange, Connect, Portal, and integrated assessment tools — using iPad IAM assessments, shared life plans, and a data exchange to consolidate information across providers. The MediSked system shifted PHP to a goal‑based, person‑centered model with real‑time reporting and cross‑agency data sharing, producing better care at a lower overall cost, significantly increased revenue visibility, and a successful nationwide DD‑centric rollout.
