MEDHOST’s YourCare Community gives patients a single view of their care—including lab and test results—and helps providers streamline care coordination and interoperability; Mt. San Rafael was an early beta tester. The hospital reports consistent portal enrollment and patient communication open rates of 30–50%, and supported adoption with a Patient Portal Navigator, kiosks, QR codes and a hotline—delivering a more informed, engaged community and increased patient trust.
