Christiana Care Health System, a large Delaware-based provider and regional cardiology center, needed a stronger population health management approach to improve outcomes for patients with ischemic heart disease and reduce avoidable readmissions and costs. To support its “Bridges” care and management program, the health system worked with Medecision and its Aerial™ platform to better coordinate care across hospital, outpatient, and community settings.

Medecision helped Christiana Care integrate disparate clinical data, automate risk-based stratification, and streamline care manager workflows so teams could prioritize higher-risk patients and intervene more effectively after discharge. The results included $1.7 million in annual savings, a care manager ratio improvement from 1/2,000 to 1/2,500, a 50% reduction in readmission process steps, a 43% drop in overdue tasks, and strong patient satisfaction scores, with 91% saying they received the right amount of follow-up.
