Peak Resources, a family-owned operator of seven skilled nursing and rehabilitation facilities in North Carolina, faced the industry shift from fee-for-service to value-based bundled payments under CMS Model 3. To manage resident-level costs, reduce readmissions, and coordinate care with hospitals and post-acute partners, Peak partnered with MatrixCare and implemented MatrixCare’s EHR with embedded Clinical Decision Support, Observations and Events tracking, and bundled-payment workflows.

MatrixCare’s solution provided real-time CDS alerts, protocol-driven interventions, and interoperability (including API integration and MatrixCare DIRECT), enabling Peak to monitor five high-risk diagnoses and manage episodes of care more proactively. The outcome: improved relationships with hospitals and other providers, streamlined documentation and workflows, and measurable impact — for example, of 30 Medicare admissions in one month only two returned to the hospital — demonstrating reduced 30-day readmissions and greater operational efficiency driven by MatrixCare.
