Benedictine Health System (BHS) needed to enable electronic exchange of Continuity of Care Documents (CCDs) with acute-care partners like Allina to improve admissions, transitions and timeliness of clinical data, but faced common interoperability barriers (disparate systems, security, standards and limited LTPAC funding). BHS worked with vendor MatrixCare, Allina and the Minnesota Department of Health (via an eHealth Connectivity Grant) to scope a practical, standards-based CCD exchange between BHS’s MatrixCare software and Allina’s EpicCare EHR.

MatrixCare implemented structured, discrete-data CCD exchange so admissions staff can request and receive on-demand CCDs into MatrixCare (and Allina can pull CCDs from MatrixCare), replacing fax/PDF workflows. The MatrixCare-enabled solution delivered timelier, more complete clinical information to drive decisions, streamlined admissions/discharge processes, and is being extended to additional sites—creating a repeatable connection any MatrixCare customer can leverage to exchange CCDs with Allina locations and other EHRs/HIEs.
