Princeton HealthCare System Reduces Inpatient Admissions 15% using Care Management

Princeton HealthCare System, a 355-bed health system in Plainsboro, NJ, faced the challenges of moving to value-based care—managing multiple payer contracts, coordinating care across different EHRs, and closing gaps in quality while controlling costs. To address these issues, Princeton partnered with Lightbeam Health Solutions and implemented Lightbeam’s Population Health Management Platform to combine disparate data sources and support new care-management workflows.

Using Lightbeam Health Solutions’ platform—including risk stratification (Johns Hopkins ACG and Lightbeam’s ATI score), a Cohort Builder, a Care Management workflow engine, and Quality Measure Optimization—Princeton automated patient identification, assigned care managers, and tracked evidence-based care plans. The initiative produced measurable results: inpatient admissions fell 15% (across MSSP and commercial programs), readmissions dropped 6%, inpatient spend declined 4%, and Princeton achieved the highest quality tier in a commercial contract.
