Maury Regional Health Building a Population Health Infrastructure from Scratch

Maury Regional Health, a medium-sized rural health system in Columbia, Tennessee, faced a major challenge after joining the Medicare Shared Savings Program: it lacked the population health infrastructure and data-driven workflows needed to shift from reactive, encounter-based care to proactive population health management. To close gaps and redesign care management, Maury reorganized its care teams, hired a vice president of population health, and contracted with Lightbeam Health Solutions to provide a Population Health Management Platform and enterprise data warehouse integrated with its Cerner EHR.

Lightbeam Health Solutions supplied integrated claims and EHR data, analytics, and patient registries that let Maury identify high-risk patients, close care gaps, and assign populations to care managers. Armed with Lightbeam’s insights, Maury hired six care managers, deployed ambulatory and transitional care workflows, and began reducing readmissions and utilization; in the first year the ACO generated over $560,000 in MSSP savings, and costs for MSSP/MA patients have since started to decline while care coordination efforts expand (including BPCI-A participation, NCQA PCMH plans, telehealth, and remote monitoring).
