Western State Medicaid Program Leverages Real-world Data to Accurately Identify At-risk Individuals

Western State’s Medicaid program struggled to identify and prioritize people at risk of homelessness because relevant information was scattered across siloed systems, often incomplete for transient or high‑risk individuals, and lacked a longitudinal view of social risk factors. The program’s goal was to reduce healthcare costs and improve outcomes by connecting those experiencing or at risk of housing instability to timely supportive services.

The state partnered with LexisNexis Risk Solutions to link public, proprietary and credit‑based SDOH data with Medicaid, HMIS, hospital, foster care and criminal‑justice records, producing a Housing Stability Score to target outreach. With frequent data refreshes and advanced matching, the initiative significantly improved identification accuracy (state+LexisNexis +6.56% vs state only; LexisNexis only +3.28%), delivered a more holistic view of need, aligned preventive interventions, and created a scalable model now moving into Phase 2.
