Using Social Determinants of Health Data to Improve Prediction and Stratification for Medication Adherence and Management

A global healthcare organization that manages healthcare solutions, logistics and supplies needed to identify patients who were non‑adherent to medications early in the year so care management could intervene. Because many patients were newly enrolled and lacked sufficient claims history, their baseline predictive models underperformed in the first months, limiting timely outreach and efficient resource allocation under value‑based care pressures.

To address this, the organization layered LexisNexis® Socioeconomic Health Attributes and Scores (SDoH) into their baseline clinical and claims models. The comprehensive models improved early‑year performance substantially—recall rose about 20 percentage points in January and remained about 10 points higher by Q2, while precision improved roughly 5% early on—enabling better targeting, reduced wasted outreach, earlier intervention opportunities, and the potential for improved outcomes and cost savings.
