Symbria Using Process Automation to Reimagine Patient Care

Symbria, a provider of rehabilitation, wellness and pharmacy services for senior living and post-acute care providers, faced a time-consuming, manual workflow for “plan of care” documents that required physician signatures within 30 days to secure reimbursement. Rehab program managers had to print, carry or fax forms, chase signatures, then scan and upload signed documents — a process that diverted therapists from patient care and created administrative burdens and denial risk.

By implementing Laserfiche forms and automated routing, Symbria digitized the plan-of-care workflow: forms are uploaded, read for physician registration data, barcoded, and sent for signature, then verified and uploaded to therapy software and medical records by data-entry staff. The change removed most manual steps for therapists, saving individual managers about 2–5 hours per week, improved scalability across 150+ clients, and paved the way for further digital automation in HR and accounts payable.
