How Symbria Inc. Uses Laserfiche to Streamline Plans of Care for Patients

Symbria Inc., a provider of therapy services to hundreds of skilled nursing facilities, faced a time-consuming, paper-heavy process for obtaining physician signatures on patient plans of care—documents that must be physician‑approved within 30 days or risk reimbursement denial. Rehab Program Managers spent significant time printing, faxing, hand-delivering, scanning and tracking unsigned forms across sites that process anywhere from a few to over 100 documents weekly, creating administrative burden and compliance risk.

Symbria implemented Laserfiche Forms and Workflow to automate the process: managers upload plans of care to a form, Workflow applies a barcode, runs OCR and validation checks (physician NPI, patient name, discipline, date), digitally faxes to physicians, imports signed returns via an import agent and routes tasks and reminders to corporate staff for follow‑up. The automation shifted administrative work away from therapists, improved accuracy and compliance controls, and saves each facility an average of five hours per week while maintaining strict document access controls.
