Grady Health System, one of the nation’s largest public academic health systems and a critical safety-net provider in the Southeast, faced high 30-day readmission risk for congestive heart failure (CHF) patients complicated by multiple clinical and socioeconomic factors. To address this, Grady contracted with Jvion and deployed Jvion’s Cognitive Clinical Success Machine (an Eigen-based cognitive machine technology) to better predict and prevent CHF rehospitalizations.

Jvion’s solution analyzes thousands of local variables and has outperformed traditional predictive models, surfacing patients on a trajectory toward risk and recommending targeted, efficient clinical actions. In the seven months after deployment, clinicians were better able to identify and support high-risk CHF patients, reducing the risk of re-hospitalization and improving patient-centered care.
