Health First, Central Florida’s largest fully integrated delivery network (four hospitals, two insurance plans and outpatient/wellness services operating on the Allscripts EHR), faced high all-cause 30‑day readmissions and potential penalties. To hit its target of a 10% reduction in 30‑day readmissions, Health First partnered with Jvion and deployed Jvion’s Cognitive Clinical Success Machine (CCSM) to identify high‑risk patients and guide targeted interventions.

Jvion’s CCSM was integrated into inpatient and post‑discharge workflows—used by transitional care navigators, inpatient case managers, care navigation specialists and CCN case managers—to flag at‑risk patients, recommend individualized actions, and monitor patients for 30 days. The program delivered nearly $2M in material savings, voided more than 800 unnecessary hospital days, reduced readmissions among vulnerable patients, and positioned Jvion as the integrator for patient transitions.
