Central Florida Integrated Delivery Network, the largest provider in the area and a leader in value-based care, needed a powerful way to pinpoint patients at risk of inpatient readmission and guide effective discharges. They turned to Jvion and its Cognitive Clinical Success Machine to provide patient-level readmission risk propensities and intervention guidance.

Jvion deployed its EigenSpace/EigenSphere–based Cognitive Clinical Success Machine, which uses existing (even incomplete) data to produce precise risk scores and clinical recommendations. In month one the system flagged 175 high‑risk patients; at an average readmission cost of $11,200 this represents potential one‑month savings of more than $1.9M, and Jvion’s platform proved three times more precise than a LACE score even without historical tuning, with effectiveness expected to improve as more data is processed.
